Important Notes

  •   • You must wait 30 days after the shipment date to submit a claim for a LOST package.
  •   • All claims must be submitted within 90 days of the shipment date.
  •   • The package recipient should hold the damaged item in the event they are requested during the claim process. Failure to retain damaged property could affect final settlement of the claim.
  •   • You must upload the Invoice/Itemized Order and Confirmation of loss/damage in order to submit your claim.
  •   • WARNING: Any fraudulent claims will make the shipper and/or package recipient liable for prosecution of mail fraud under the Federal Criminal Code.

2. Upload a copy of the invoice or itemized order detail for this shipment.

3. Upload a copy of the original message or email from the package recipient confirming the loss or damage you are claiming.

4. Acknowledge, sign and submit the form.
